2012 Spring Course – Registration NOW OPEN!

2012 Spring Course materials will be available March 10th  for downloading (pending timely submission by speaker).  Agenda, times, or objectives are subject to change. 

Registration Instructions 

IMPORTANT:  A registration form is required for all participants! 
Please print and mail form to Janet Lemon (mailing address on registration form).

1.  Fill out registration form and submit by traditional mail.  Click here for form.

 2.  Remit conference payment using online service (see instructions below) or send check via mail.   Instructions for mail payment are on the registration form.

Online Payment 

The name on your registration form must match the billing name on the payment form or your registration may not be properly processed.  After clicking on the appropriate fee below, you will be directed to the PayPal website. Late fee of $15 applies after March 17, 2012.

You may either:
- Select a PayPal account to pay for the conference fees. You will be asked to log into the account.
- Select the “Pay with debit or credit card” option to pay for conference fees using any major credit or bank card. You will not be forced to register with PayPal to make a payment.

SGNA member     $90.00

Non member    $150.00

Technicians/LVNs    $75.00

Group discount (5 or more members from the same hospital)  No PayPal option
  $5 off member and LVN/tech rate
  $10 off non-member rate
